We collaborate closely with hospital teams to ensure a complete, safe, and well-organized discharge. From medication support to follow-up planning, Finding home health or Homecare service, & Placement to ALFs. We eliminate guesswork and empower a strong recovery.

Home Health & Private Duty Caregiver Referral Services

Wether the patient is at an Assisted Living Facility or at home. We help facilitate the referral to the right Home Health or Private Duty Home Care Agency in our network. No need to call multiple agencies to see if they would accept the patients insurance. We will do that for you.

Medication Reminder Program & Management

All patients that goes through our transition program will be provided a 30 day Medication Reminder via Daily Calls and/or Text Messages. Medication Noncompliance is the top reason for Hospital readmission and no other agency provides this service

Errors and omissions are common during discharge. Our team ensures medications are correct, understood, and taken safely. We also provide medication pickup at the pharmacy for the patients when needed to make sure they dont miss any doses.

Wound Care and Skin Assessment at Home

We work with several amazing Wound Care Companies where they will send Nurse Practitioners who are Wound Care Certified to provide treatment and recommend the correct treatment with the patients care providers

Mobile Physician & Nurse Practitioners

If the patient can’t go the Doctors Office, We can have someone go to them at home or at Assisted Living Facilities to handle the Care Plan Oversite and coordinate with the patients Primary Care Physician Directly. Virtual Doctor/NP visits are also available for the patient
